Care With Dignity provides nurse-led in-home aged care for older Australians across Melbourne's South Eastern Suburbs. Our care is funded through the Australian Government's Support at Home program, in partnership with Trilogy Care, or privately for those who prefer to self-fund.

What is Support at Home?

Support at Home is an Australian Government funding program that helps older Australians live safely and independently in their own homes. It covers everyday needs like cleaning, showering, and getting to appointments. To access your Support at Home funding, you'll need an approved provider to manage your Government subsidy.

Our partnership with Trilogy Care

Care With Dignity has partnered with Trilogy Care, an approved Support at Home provider, to give you access to quality home care services funded through your Support at Home budget. Trilogy Care manages your care package, while you keep the freedom to choose your service providers, including Care With Dignity.

Both organisations believe in putting you in control. We're here to help you get the most from your care hours and wellbeing by giving you choice, control, and independence.

What this means for you

Working with Care With Dignity and Trilogy Care means more of your Support at Home budget goes towards the care and services you need, and less towards management fees. That means you can stay at home longer with the support that matters most.

Trilogy Care keeps fees transparent and low at 10% across the board, so you get more care hours for your money.

What is self-management?

Self-management puts you in the driver's seat. It lets you choose your service providers, design your own care plan, and decide how your Support at Home budget is spent. You get real control over your care.

Switching providers

Already with Another Provider? Switching Is Easier Than You Think.

Your Support at Home funding belongs to you, not your provider. If your current provider is not delivering, you have every right to change. There is no penalty and no waiting period. We handle the transition and make sure nothing falls through the cracks.

  • A different carer every visit, no warning, no pattern
  • Visits cancelled at short notice or no-shows
  • Nobody answers the phone when something goes wrong
  • Invoices that do not match what was delivered

Common Questions About In-Home Aged Care

Do I need to have a Support at Home package already?

No. If you are not yet approved, we can help you understand the My Aged Care process and what to expect. If you already have a package, you can switch to us straight away. We also offer private care.

What if my parent needs more care than the package covers?

We will be upfront about what your funding level covers and work with you to make the most of it. Private top-up care is also available.

Do you offer private care?

Yes. We offer private care for clients who are not funded through Support at Home, or who want to supplement their funded care.
